I too DIDN'T get the email so used the link from the club website to fill mine in, but then also got the "survey completed" message when I tried to fill it in for the wife. The easiest way to get around this is to open a 'New Inprivate' window on your device (click on the 3 dots in the top right hand corner of your screen, then click on the link). This blocks the cookies from Survey Monkey and allows you to complete it again
